Use this form to request that your account be deleted from the Commissioned app. By removing your account from our systems, you will lose access to the Commissioned app that require an account. This action cannot be undone. You must include the email address linked to your Commissioned app account. We only complete requests on the information linked to your email address. To verify your identity, we will send an email to the email address you provide which matches our records and wait for your response. In some instances, we may ask for additional information to ensure we verify your identity before complying.
